Population Health Management

We are working on a project to trial a new way of consulting - Virtual Group Consultations - for some of our patients with multiple long-term conditions. This project is being led by Will Shipp one of our Social Prescribing Link Workers. We will be gathering eligible patients in virtual groups for some peer support and expert input to try to help them better manage their health. The sessions are scheduled to start later this year.
