Huge congratulations to Lynn Wrathall whose personalised care project to address clinical inequality of diabetes care for housebound patients within our practices has been awarded funding by the Queen’s Nursing Institute.
From General Practice to Housebound Diabetes Care – Mind the Gap

As the Diabetes Lead Nurse Practitioner at Whiteladies Medical Group (WMG) and Pembroke Rd (PR) Practices, Lynn identified a gap in care and approached the QNi for funding and management support.
The project outcome is to support housebound patients with diabetes at WMG and PR by providing care from the practice nurses for routine review and regular follow-up with home visiting.
